Change of address or change of name
If you have moved and need to change your address, phone or email, or you have legally changed your name; you must complete the Change of Address/Change of Name form. For change of name requests, please ensure that official documentation accompanies this form to verify the legality of any change to your name (e.g. marriage certificate, driver's license, birth certificate, change of name certificate). The Registrar's office will photocopy your original documentation (U5, Room 68 in the north location or 61 Charles Street in the downtown location). It is your responsibility to keep your address and other personal information up-to-date so that information will reach you.
