Student forms

The following forms are available for download in PDF (Adobe Acrobat). Please read each form carefully for important instructions, processing times and fees, where applicable. If you have a question about a form, please contact the Registrar’s office at 905.721.3190 or connect@uoit.ca.

Note: Forms with an asterisk (*) must be submitted according to strict deadlines. Please read the deadline information carefully on the form(s). Failure to submit a form by the deadline indicated may result in your request or application being denied.

Academic Consideration
Use this form in a situation where course work/examination is missed but is not caused by a medical reason.

Add/Drop Course (undergraduate only)
You can add a course on MyCampus online (under Add/Drop courses) only until the last day to add courses (also the last day to drop courses for a 100 per cent refund). Courses can be dropped online until the last day to drop a course in a given semester. Please refer to important dates if you are unsure of drop deadlines. This form is also used if a course is full or causes a time conflict.

Application to Graduate*
The Application to Graduate is available on your MyCampus account and is only open during specific dates, depending on the term you expect to graduate in. View the application to graduate deadlines.

Application for Deferred Final Examinations
Ths form is used for students seeking to defer a final examination. 

Authorization to Release Information
Use this form to give the university permission to release your personal information (e.g. your marks or registration information) to a third party, such as a parent.

Change of Address/Email/Phone Number
Changes to your mailing address, email or phone number must be made through your MyCampus account. After logging in, select the desired tab under Personal Information to update your address, email or phone number. 

Change of Name 

  • Legal name change: Use the Change of Name form to advise the university when you have legally changed your name. For change of name requests, please ensure official documentation accompanies the form to verify the legality of any change to your name (e.g. marriage certificate, passport, birth certificate, change of name certificate). The Registrar’s office will photocopy your original documentation (SIRC Building, at the north Oshawa location). It is your responsibility to keep your address and other personal information up-to-date so information will reach you.
  • Preferred first name change: Use the Change of Name form to submit a preferred first name change. Documentation is not required for this request; however, you must show your student ID card when submitting this form to the Registrar's office (SIRC Building, at the north Oshawa location).

Change of Program
Use this application to change your academic program. This request adheres to specific deadlines. Applications can be made through your MyCampus account under the UOIT Documents tab.

Change of Program - Management Program*
Students in the Faculty of Energy Systems and Nuclear Science, Faculty of Engineering and Applied Science or Faculty of Science must use this form to apply for the Management program.

Consent to Release Information to Student Accessibility Services
Use this form to grant the Registrar’s office permission to share your information with Student Accessibility Services as part of the admission review process. Note: This form must be used in conjunction with the Consideration on the Basis of Disability form.

Consideration on the Basis of Disability
Use this form if you believe you were not accommodated for all or part of your high school or previous diploma or degree program, or believe there are extenuating circumstances related to disability, which negatively impacted your grades. Note: This form must be used in conjunction with the Consent to Release Information form. Learn more about minimum requirements for students with disabilities.

Consideration for Late Withdrawal
Students wishing to be considered for late withdrawal from a course or program must complete the Consideration for late withdrawal form.

Course Substitution
Use this form when you would like to substitute a course in your program.

Declaration of Specialization or Minor*
Use this form to add/remove a specialization or minor.

Drop a Course
Courses that are not officially dropped will be graded and you will be responsible for paying any fees associated with the course. Note: You cannot drop your last course online. You must complete an Add/Drop form to do this and show proof of laptop return by providing a Schedule B form. If you are dropping your last Spring/Summer course and you were full-time in both the previous Fall/Winter semesters and are returning in the fall you do not need to provide a Schedule B form. If you are leaving the university for two semesters or more, you must officially withdraw from the university.

Final Grade Reappraisal/Reconsideration*
Use this form for reappraisal/reconsideration of a final grade. It must be submitted within 10 working days after the release of the final grade.

Medical Statement*
A student may use this form in a situation where coursework/examination is missed due to a medial reason. This form may also accompany an appeal/review. The student must complete all additional forms and provide supporting documentation where appropriate. If you missed a final examination, you must also submit a Deferred final examination applicationNote: A physician/counsellor must complete, sign and date section B of the medical statement form no later than 24 hours after the scheduled examination. This form must be received by the appropriate UOIT office within three working days of the missed deadline or exam date.

Notice of Academic Appeal*
Use this form to appeal a formal decision of a Dean, including decisions relating to final grades, academic conduct or professional suitability, or any other decision made by the faculty of an academic nature. Note: This form must be submitted to the Secretary of Academic Council within 10 working days of the date of the decision.

Prerequisite Waiver/Course Overload
Use this form if you are seeking a course prerequisite/course restriction waiver or permission to overload the number of courses taken each term.

Readmission application*
Use this application if you have been out of school for two semesters or have been on academic suspension and want to return. This request adheres to specific deadlines. Applications can be made through your MyCampus account under the UOIT Documents tab.

Review of Academic Standing (review of suspension, dismissal, etc.)*
Use this form for review of your academic standing due to extenuating circumstances beyond your control that affected your academic performance. This form must be submitted to the dean of your faculty within 10 working days of the notification of academic standing.

Tax form (T2202A)
This form is available online on your MyCampus account and is required to support your claim of eligible tuition and education amounts for the year. Learn more about viewing your T2202A form on the Web.

Transcript Request
Transcript requests can be made online through your MyCampus account. After logging in, select the Request printed official transcript tab under Student Records to begin the request process.

Verification of Enrolment Request
Verification of Information requests can be made online through your MyCampus account. After logging in, select the Verification of Enrolment Letter tab under Student Records to begin the request process.

Withdrawal from the University/Program
Use this form if you are seeking a complete withdrawal from your program and the university.
