For valuable consideration, I hereby irrevocably grant to the University of Ontario Institute of Technology (“UOIT”), its respective agents in perpetuity and throughout the world the right to (i) capture and record my name, image and likeness by any mechanical or electronic mean or contrivance including without limitation by way of a photograph, electronic image, video image, “behind the scenes” footage (collectively my “Depiction”) and (ii) use, disclose, post, exhibit, license or otherwise exploit my Depiction on any medium now existing or to be invented including without limitation on any website, blog, e-blasts, in any printed or online publication, in any television program and in any radio, TV or print advertisement, the whole, for the purposes of (i) promoting UOIT and/or(ii) filming or recording stories pertaining to UOIT programs, events or other UOIT specific initiatives. I release UOIT, and those who get their rights from or through UOIT, from any claims or actions I may make arising out of the use of the Depictions. Further, I agree that UOIT will exclusively own all rights in and to the Depictions, and that UOIT shall be entitled to edit the Depictions in its sole discretion, as UOIT may deem appropriate. I hereby irrevocably waive, in favour of UOIT: (i) any and all so-called “moral rights” that may be recognized now or in the future in connection with the Depictions in any jurisdiction throughout the world; (ii) any rights that I may have to inspect or approve any of the finished or unfinished Depictions (whether or not edited in accordance with this release form); and (iii) any rights that I may have at law or in equity, including but not limited to any privacy laws applicable and in effect from time to time, to control or restrain the public use of my name and likeness. I confirm that I am not a member of any model or photographic guild or union. I agree that UOIT and its licensees and assigns may freely assign or license this release form and/or any right, licence or waiver granted herein, in whole or in part, to any third party. This is the entire agreement between us and shall be governed by the laws applicable to contracts made and performed entirely within the province of Ontario
